levels is included in a model
formula, the variable is expanded to binary indicator variables. A
separate parameter is estimated for each category. For a subject in the 
category, the indicator variable is equal to 1
and all of the other indicators are 0. Parameter names appearing on the output
summary for a model are modified to indicate the level.
